We provide a comprehensive range of disability support services tailored to meet the individual needs of each participant. Our supports are available to individuals funded through the NDIS (National Disability Insurance Scheme) as well as those accessing other funding options or private services.
Our goal is to support people living with disability to build independence, improve daily living skills, and actively participate in their community.
Community Access SupportOur community access services support participants to engage in social, recreational, educational, and community activities. This may include attending appointments, shopping, participating in hobbies, building social connections, or attending community programs. We focus on promoting inclusion, confidence, and meaningful participation.
Personal Care SupportWe provide professional and respectful personal care assistance tailored to individual needs. Support may include showering, dressing, grooming, toileting, mobility assistance, and medication reminders. All care is delivered with dignity, privacy, and in accordance with safe practice and infection control standards.
Assistance with Household TasksMaintaining a safe and organised home environment is essential for wellbeing. We assist with household tasks such as laundry, dishwashing, changing bed linen, light tidying, and general household organisation to help participants maintain independence at home.
Assistance with Daily Living SkillsOur support workers assist participants in developing and strengthening daily living and life skills. This may include budgeting, grocery shopping, meal planning, time management, personal organisation, communication skills, and establishing daily routines that promote long-term independence.
Assistance with Meal PreparationWe provide support with meal planning and meal preparation, including grocery shopping, safe food handling, cooking support, and promoting balanced nutrition. Our aim is to build confidence and independence in the kitchen while supporting overall health and wellbeing.
In-Home Cleaning ServicesWe offer in-home cleaning support to maintain a hygienic and safe living environment. Services may include vacuuming, mopping, bathroom and kitchen cleaning, dusting, and general sanitation.

How do I start the onboarding process with Inclusive Community Care?
Starting your support journey with Inclusive Community Care is simple and streamlined through our online referral process.
We welcome referrals from participants, family members, carers, support coordinators, plan managers, allied health professionals, and other service providers.
Step 1: Submit Our Online Referral Form
To begin, complete the referral form available on our website.
